If you are a client at West Roseville Veterinary Hospital and you would like to request a refill of your pet's prescription, you are welcome to complete this request form.
PLEASE NOTE: We will try to fill your prescriptions as soon as possible, but please expect a 72 hour “turn-around time” for your request to be processed. After receiving your request, a staff member will contact you by telephone regarding your prescription.
This refill request form is for the convenience of our clients that have previously been prescribed a medication by one of our veterinarians. The prescribing doctor will review the request, and you will be contacted if it cannot be filled for some reason.
If your pet is due for bloodwork or a physical exam your request may be denied. Routine bloodwork may be required to measure the levels of medication in your pet's blood and monitor organ function during the course of therapy.
Federal and State laws as well as good medical practice prohibit us from dispensing prescription medications without prior
examination of your animal, and current knowledge of your animals health.
